Piracetam is a nootropic drug belonging to the racetams group, with the chemical name 2-oxo-1-pyrrolidine acetamide. It is a cyclic derivative of the neurotransmitter gamma-aminobutyric acid (GABA), but its mechanism of action is different from that of endogenous GABA. Piracetam is known for its neuroprotective and anticonvulsant properties and is reported to improve neural plasticity. It has been documented to be effective in treating cognitive disorders and dementia, vertigo, cortical myoclonus, dyslexia, and sickle cell anemia, although its clinical application in these conditions is not yet fully established. Additionally, piracetam has effects on the vascular system by reducing erythrocyte adhesion to the vascular endothelium, hindering vasospasms, and facilitating microcirculation.
The drug is rapidly and extensively absorbed following oral administration, with peak plasma concentration typically reached within one hour after dosing in fasted subjects. The absolute bioavailability of oral piracetam formulations is close to 100%, and steady-state plasma concentrations are achieved within three days of dosing. Piracetam is not significantly bound to plasma proteins and is predominantly excreted via renal elimination, with about 80-100% of the total dose recovered in the urine.
Piracetam has been studied for its potential cognitive-enhancing effects, and it is believed to help maintain the membrane flexibility of cells in the brain and blood vessels, which can become stiff during aging and in some diseases. It is also used to improve brain function and has been investigated for its effects on Alzheimer's disease, autism, dementia, depression, memory, schizophrenia, and sickle cell disease, among other conditions. However, evidence to support its use for many of these conditions is not conclusive.
