Mefenamic acid, available under the brand name Ponstel, is a nonsteroidal anti-inflammatory drug (NSAID) that is primarily used for the short-term treatment of mild to moderate pain. It is also used to alleviate menstrual pain. Mefenamic acid belongs to the class of drugs known as fenamates and works by inhibiting the cyclooxygenase (COX) enzymes, COX-1 and COX-2, which are responsible for the production of prostaglandins that cause inflammation, pain, and fever.
The drug is typically administered orally and should be taken with a full glass of water. It is important to follow the prescribed dosage and not to exceed the recommended duration of therapy, which is usually not more than 7 days. The usual adult dose for pain relief starts with an initial dose of 500 mg, followed by 250 mg every 6 hours as needed. For dysmenorrhea, the same initial dose is taken, followed by 250 mg every 6 hours for 2 to 3 days.
Mefenamic acid can cause side effects, with the most common being gastrointestinal issues such as abdominal pain, constipation, diarrhea, and nausea. More serious side effects can include increased risk of cardiovascular thrombotic events, such as myocardial infarction and stroke, and gastrointestinal events like bleeding, ulceration, or perforation of the stomach or intestine. It is contraindicated in individuals with a history of asthma, urticaria, or other sensitivity reactions to aspirin or other NSAIDs, and in those who have had coronary artery bypass graft (CABG) surgery.
