Desvenlafaxine succinate, marketed under the brand name Pristiq among others, is a selective serotonin and norepinephrine reuptake inhibitor (SNRI) used for the treatment of major depressive disorder (MDD). It is an active metabolite of the antidepressant venlafaxine. As an SNRI, desvenlafaxine works by increasing the levels of serotonin and norepinephrine in the brain, which helps to improve mood and alleviate symptoms of depression.
Mechanism of Action: Desvenlafaxine exhibits its therapeutic effects by potentiating the neurotransmitters serotonin and norepinephrine in the central nervous system through the inhibition of their reuptake. It has a higher affinity for serotonin transporters compared to norepinephrine transporters and shows the least affinity for dopamine transporters.
Dosage and Administration: Desvenlafaxine is available in extended-release tablet form and should be taken once daily. The dosage is typically started at a low level and may be increased based on the patient's response and tolerance. It is important to follow the prescribing physician's instructions regarding dosage and to monitor for potential side effects.
Side Effects: Common side effects may include nausea, dizziness, dry mouth, sleep disturbances, and constipation. More serious side effects can occur, such as increased suicidal thoughts in children and young adults, serotonin syndrome, and hyponatremia, particularly in the elderly.
Precautions: Desvenlafaxine should not be used in patients who are hypersensitive to the drug or to venlafaxine. It should also be used with caution in patients with a history of mania, seizures, liver or kidney disease, and those at risk for bleeding. The drug is not recommended for use during pregnancy or breastfeeding due to potential risks to the fetus or infant.
