Tacrolimus, also known by the brand name Prograf, is an immunosuppressive medication primarily used to prevent organ rejection in patients who have undergone organ transplants, such as kidney, liver, heart, or lung transplants. It is a macrolide antibiotic that was originally isolated from the bacterium Streptomyces tsukubaensis.
Mechanism of Action: Tacrolimus works by suppressing the immune system's response to a foreign organ. It binds to a protein called FK-binding protein 12 (FKBP-12) and the resulting complex inhibits the enzyme calcineurin. This inhibition prevents the activation of T-cells, which are key players in the immune response against the transplanted organ, thus reducing the risk of rejection.
Indications:
Dosage and Administration: The dosage of tacrolimus is highly individualized and depends on the patient's medical condition, the type of transplant, and the patient's response to therapy. It is usually initiated at a higher dose immediately following transplantation and then adjusted over time based on blood level monitoring. Tacrolimus is available in oral formulations, such as capsules and tablets, as well as an injectable form for intravenous use.
