Metamizole, also known as dipyrone, is a pharmaceutical drug that is classified as a nonsteroidal anti-inflammatory drug (NSAID) and is primarily used as an analgesic and antipyretic. It is a potent pain reliever and fever reducer, and it has spasmolytic properties, which can help to relieve muscle spasms. The drug is used to treat severe and persistent fever and pain, including conditions such as postoperative pain, gastrointestinal pain, cancer pain, and migraine headaches.
The mechanism of action of metamizole is not fully understood, but it is believed to involve the inhibition of prostaglandin synthesis, similar to other NSAIDs. However, it is not classified as a typical NSAID and is thought to have a different mechanism, possibly involving the inhibition of COX-3 rather than COX-1 or COX-2. Metamizole is rapidly metabolized in the body to its active form, 4-methyl-amino-antipyrine (MAA), which has been shown to inhibit prostaglandin E2 (PGE2)-induced hyperalgesia.
Despite its therapeutic uses, metamizole has been associated with potentially fatal agranulocytosis, a severe blood disorder that results in a dangerously low neutrophil count.
