Calcipotriene (Calcipotriol)

Chemical and Pharmacological Properties

  • Chemical Name: (5Z,7E,22E,24S)-24-Cyclopropyl-9,10-secochola-5,7,10(19),22-tetraene-1α,3β,24-triol

  • Mechanism of Action: Synthetic vitamin D3 analog that binds to vitamin D receptors (VDR) in keratinocytes and immune cells. It regulates cell differentiation and proliferation while reducing inflammation.

  • Bioavailability: Minimal systemic absorption when applied topically.

Indications

  • Primary Use: Treatment of plaque psoriasis (psoriasis vulgaris).

  • Off-label Uses:

    • Scalp psoriasis

    • Mild-to-moderate chronic plaque psoriasis in adults and children (in some formulations)

Dosage and Administration

  • Topical cream/ointment/scalp solution: Apply a thin layer once or twice daily to affected areas.

  • Maximum weekly dose: 100 g (to avoid hypercalcemia risk).

  • Combination therapy: Often used with corticosteroids for enhanced efficacy.

Adverse Effects

  • Common:

    • Skin irritation, burning, itching, or dryness at the application site.

    • Mild erythema (redness) or peeling.

  • Serious (rare):

    • Hypercalcemia (elevated calcium levels) with excessive use.

    • Worsening of psoriasis (paradoxical reaction).
