Trilostane is a synthetic steroid that acts as a competitive inhibitor of the enzyme 3β-hydroxysteroid dehydrogenase (3β-HSD), which is essential for the synthesis of cortisol and other steroids. By inhibiting this enzyme, trilostane reduces the production of glucocorticoids and, to a lesser extent, mineralocorticoids and sex hormones. It is primarily used in veterinary medicine for the treatment of hyperadrenocorticism (Cushing's disease) in dogs and cats.
Mechanism of Action: Trilostane inhibits the adrenal cortex's ability to produce cortisol, the main glucocorticoid in dogs and cats. It does this by blocking the enzyme 3β-HSD, which is necessary for the conversion of pregnenolone to progesterone, a key step in the synthesis of various steroid hormones. This results in decreased levels of cortisol, which can help manage the clinical signs associated with hyperadrenocorticism.
Indications: Trilostane is indicated for the long-term treatment of pituitary-dependent hyperadrenocorticism (PDH) and adrenal tumor hyperadrenocorticism (ATH) in dogs. It is also used off-label for the treatment of alopecia X in dogs and has been reported to be effective in some cats with hyperadrenocorticism.
