Captopril is an angiotensin-converting enzyme (ACE) inhibitor used for the management of essential or renovascular hypertension, congestive heart failure, left ventricular dysfunction following myocardial infarction, and nephropathy, including diabetic nephropathy. It works by inhibiting the conversion of angiotensin I to angiotensin II, a potent vasoconstrictor, thereby reducing blood pressure and afterload on the heart. Captopril is also used to improve survival and reduce the risk of heart failure in patients with left ventricular hypertrophy post-myocardial infarction and to treat kidney disease caused by diabetes in patients with type 1 diabetes and retinopathy.

Captopril is supplied in oral tablet form with various strengths, typically starting at 12.5 mg to 25 mg three times a day and can be increased up to a maximum daily dose, usually not exceeding 450 mg. It is important to take Captopril on an empty stomach, at least one hour before meals, to ensure optimal absorption.

Common side effects of Captopril include a dry, hacking cough, dizziness, lightheadedness, and a change in the sense of taste. More serious side effects can include angioedema, neutropenia, kidney damage, hyperkalemia, and liver damage. Captopril is classified by the FDA as pregnancy category D, indicating that it can harm a fetus, and should not be used during pregnancy.

Patients with a history of angioedema associated with previous ACE inhibitor treatment or hereditary angioedema should not use Captopril. It is also contraindicated in patients who are hypersensitive to Captopril or any other ACE inhibitors. Captopril should be used with caution in patients with renal impairment and in those who are volume or salt-depleted, as it may cause excessive hypotension.

When stored, Captopril should be kept at room temperature, away from moisture and heat, and protected from light. It is crucial for patients to report any signs of infection, as Captopril can suppress the immune system and increase the risk of infections.
