Demecarium bromide, also known as demecarium, is an indirect-acting parasympathomimetic agent that has been used in the treatment of glaucoma. It functions as a cholinesterase inhibitor, which means it prolongs the effect of acetylcholine by inhibiting the enzyme cholinesterase that would otherwise break it down. This leads to constriction of the iris sphincter muscle, causing miosis, and contraction of the ciliary muscle, which facilitates the outflow of aqueous humor and results in a reduction of intraocular pressure.
The drug is a long-acting cholinesterase inhibitor and potent miotic, which makes it useful for the topical treatment of chronic open-angle glaucoma. However, due to its toxicity, it is generally reserved for cases that cannot be adequately controlled with shorter-acting miotics or other agents. Demecarium bromide is known to produce intense miosis and ciliary muscle contraction, which is accompanied by increased capillary permeability of the ciliary body and iris, increased permeability of the blood-aqueous barrier, and vasodilation.
Adverse effects of demecarium bromide can include symptoms such as nausea, vomiting, abdominal cramps, diarrhea, urinary incontinence, salivation, sweating, difficulty in breathing, bradycardia, or cardiac irregularities in case of overdose. It is important to use this medication under the supervision of a healthcare provider and to be aware of the potential side effects and interactions with other medications.
