Losartan potassium, marketed under the brand name Cozaar, is an angiotensin II receptor blocker (ARB) primarily used to treat hypertension. It is also utilized to reduce the risk of stroke in certain patients with hypertension and left ventricular hypertrophy, and to treat diabetic nephropathy in patients with type 2 diabetes and hypertension. Losartan works by blocking the action of angiotensin II, a hormone that causes blood vessels to narrow, thereby allowing blood to flow more smoothly and the heart to pump more efficiently.
The drug is available in oral tablet form and is typically taken once or twice daily. It is also available in combination with hydrochlorothiazide, a diuretic, under the brand name Hyzaar. Losartan has a long duration of action, with once-daily administration being effective for 24 hours. It is important to monitor blood pressure regularly while taking this medication, as it may take several weeks to achieve the full blood pressure-lowering effect.
The absorption of losartan is approximately 33%, and its active metabolite, E-3174, is formed in the liver through metabolism by cytochrome P450 enzymes, primarily CYP2C9. The drug and its metabolite are highly protein-bound in serum and have elimination half-lives of 1.5-2.5 hours and 6-9 hours, respectively.
