Policosanol is a natural mixture of long-chain aliphatic alcohols that is primarily isolated and purified from sugar cane wax, with octacosanol being the main component. It is also found in other plant sources such as beeswax, wheat germ oil, and rice bran. Policosanol has been the subject of numerous studies, particularly in the 1990s, which suggested it could have lipid-lowering effects, potentially by affecting the enzyme activity of HMG-CoA reductase, although this mechanism has not been conclusively proven.
The lipid-lowering effects of policosanol have been reported in various patient populations, including healthy volunteers, individuals with hypercholesterolemia, diabetics, and postmenopausal women. Early studies, many conducted in Cuba, indicated that policosanol could significantly reduce levels of total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), and triglycerides (TG), while increasing high-density lipoprotein cholesterol (HDL-C). However, subsequent studies conducted outside of Cuba have not always replicated these findings, leading to a debate over its efficacy.
Policosanol is generally well tolerated, with mild short-term side effects reported in some trials. It has also been reported to have antiplatelet aggregation effects, suggesting that it should be used with caution in combination with other antiplatelet or anticoagulant agents.
