Meloxicam is a nonsteroidal anti-inflammatory drug (NSAID) used to treat various conditions such as osteoarthritis, rheumatoid arthritis, and juvenile rheumatoid arthritis in pediatric patients. It is available under brand names including Anjeso, Mobic, Qmiiz, Vivlodex, and Zynrelef. Meloxicam is known for its longer half-life compared to other NSAIDs, which allows for once-daily dosing. It is available in oral, transdermal, and intravenous forms and is considered a preferential COX-2 inhibitor, which may reduce the risk of gastrointestinal side effects, although this is a topic of debate .
The drug's mechanism of action involves inhibiting both cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes, which are responsible for the production of prostaglandins that cause inflammation and pain. Meloxicam's preferential inhibition of COX-2 contributes to its anti-inflammatory effects while reducing the risk of gastrointestinal irritation .
Meloxicam has a half-life of approximately 20 hours, which is longer than most other NSAIDs, allowing for once-daily dosing. It is metabolized primarily by the cytochrome P450 enzyme CYP2C9, with minor contributions from CYP3A4. The drug is highly protein-bound, mainly to albumin, and is eliminated through both renal and fecal routes .
Common side effects of meloxicam may include gastrointestinal issues such as nausea, vomiting, and abdominal pain. More serious side effects can involve the cardiovascular and renal systems. Meloxicam should be used with caution in patients with a history of gastrointestinal ulcers or bleeding, heart failure, or kidney disease.
