Quetiapine Fumarate, known by the brand name Seroquel among others, is an atypical antipsychotic medication used to treat schizophrenia, bipolar disorder, and major depressive disorder. It works by helping to restore the balance of certain natural substances in the brain (neurotransmitters).
Mechanism of Action: Quetiapine is a selective antagonist of serotonin (5-HT2) and dopamine (D2) receptors, which are believed to play a role in the positive and negative symptoms of schizophrenia. It also has some activity against the histamine H1 and adrenergic α1 receptors, which may explain its sedative and orthostatic hypotensive effects.
Pharmacokinetics: The drug is well absorbed after oral administration, and its bioavailability is not significantly affected by food. Quetiapine is extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily by the isoenzyme CYP3A4, and it also inhibits the CYP1A2 enzyme to a lesser extent.
Dosage and Administration: The usual starting dose for schizophrenia is 25 mg twice daily, with increases based on clinical response and tolerability. For acute manic episodes associated with bipolar disorder, the initial dose is typically 100 mg daily, which can be increased up to 800 mg daily. Dosages may need to be adjusted in patients with hepatic or renal impairment.
