Artesunate is an artemisinin derivative used for the initial treatment of severe malaria. It is rapidly metabolized to its active form, dihydroartemisinin (DHA), which is responsible for its antimalarial effects. The drug works by generating free radicals that inhibit the normal function of Plasmodium parasites, thereby combating the erythrocytic stages of the malarial infection. The World Health Organization recommends artesunate as the first-line treatment for severe malaria, and it was granted FDA approval on May 26, 2020 .
Artesunate has a short duration of action due to its short half-life and a moderate therapeutic index. It is important to note that artesunate does not treat the hypnozoite liver-stage forms of Plasmodium, thus it does not prevent relapses of malaria caused by P. vivax or P. ovale. Therefore, concomitant therapy with another antimalarial agent, such as an 8-aminoquinoline drug, is necessary for treating severe malaria caused by these species .
The drug is typically administered intravenously as a slow bolus over 1-2 minutes and should not be given via continuous IV infusion. It is also available in tablet form for oral administration. Artesunate should be used with caution in patients with known hypersensitivity, as it can cause severe allergic reactions, including anaphylaxis
